Your current W-9 for payment and 1099 reporting. - NOA (if applicable)
Notice of Assignment from your factoring company, so we pay the right party. - COI listing us as Certificate Holder
Certificate of Insurance listing Conard Logistics, 200 International Blvd, La Vergne, TN 37086 as Certificate Holder and Additional Insured. Your agent can send it to [email protected].
